29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:    The employer does not protect each employee using a pneumatic nail gun from eye injuries by using ANSI Z87.1 rated eye protection.       This most recently occurred on May 16, 2013, at 220 S. Douglas, Belleville, IL. Employees installing shingles with a pneumatic nail gun were not wearing ANSI Z87.1 rated eye protection. The employer did not provide his employees with eye protection while using a pneumatic nail gun.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     The employer does not protect each employee using the extension ladder to access work on the roof exposing employees to fall hazards. The ladder is not extended three feet above the upper landing surface nor were there any grasping device, such as a grab rail to assist employees in mounting and dismounting the ladder.      This most recently occurred on May 16, 2013, at 220 S. Douglas, Belleville, IL. An extension ladder to access work on the roof did not extend three feet above the upper landing surface nor were there any grasping device, such as a grab rail to assist employees in mounting and dismounting the ladder. Employees were exposed to fall hazards of 12 feet 9 inches to a lower level.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b):    The employer does not protect each employee from falls from roofs by an adequate fall protection system.       This most recently occurred on May 16, 2013, at 220 S. Douglas, Belleville, IL. Employees working from a 6 in 12 pitch roof are exposed to falls of approximately 12 feet 9 inches to a lower level. Employees are not protected from falling by guardrail systems, safety net systems, or a personal fall arrest systems or any other approved means.      Affordable Roofing & Exteriors, Inc., was previously cited for a violation of this occupational safety and health standard or an equivalent standard contained in:       OSHA   Inspection number 423703, citation 2 item 1 issued on June 15, 2012, with respect to a workplace located at 200 E.
