29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented:      Each employee on a 4 in 12 pitch and a 7 in 12 pitch roof is not protected from falls from roofs by an adequate fall protection system.      Employees working from a 4 in 12 pitch and 7 in 12 pitch roof are exposed to falls of approximately 9 foot and 20.4 foot to a lower level respectively.  Employees are not protected from falling by guardrail systems, safety net systems, or a personal fall arrest system or any other approved means.      Affordable Roofing & Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard or an equivalent standard contained in:      OSHA Inspection number 423703, citation 2, item 1 issued on June 15, 2012 and became final order on or about June 28, 2013, with respect to a workplace located at 200 East Cedar Street, New Baden, Illinois 62265.      OSHA Inspection number 313016370, citation 1, item 3 issued on August 13, 2010 and became final order on or about December 22, 2010, with respect to a workplace located at 2508 Hunters Point Boulevard, Edwardsville, Illinois 62025.      OSHA Inspection number 313012999, citation 1, item 2A issued on August 31, 2009 and became final order on or about September 24, 2009, with respect to a workplace located at Weatherby Road and Hunters Trail, Mascoutah, Illinois 62258.      Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect the employees from fall hazards.
