29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required. The employer failed to provide documentation demonstrating that the following cited hazards were abated: 1. Employees working from a residential roof installing shingles at a height greater than six feet were not wearing fall protection. 2. Individuals were using a portable ladder that did not extend three feet above the landing surface of the rooftop nor was the ladder secured to prevent displacement.

Bulldog Construction was previously cited for a violation of this or its equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number 313133720, Citation Number 1, Item Number 1, Issued on 6/29/2010, and was affirmed final order on 8/6/2010, with respect to a workplace located at 400 Giertson, Stoughton, WI 53589.
