29 CFR 1910.212(a)(1): The employer did not ensure that one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards such as those created by the unguarded portion of the saw blade.  a.) Tool Room - On or about May 17, 2013, and at times prior thereto, employees were potentially exposed to the unguarded saw blade during use of the Jet brand horizontal band saw.  No adjustable guard was installed to cover the unused portion of saw blade.

29 CFR 1910.215(a)(4): The employer did not ensure that the work rest on offhand grinding machines was kept adjusted closely to the wheel with a maximum opening of one-eighth inch.  a.) Tool Room - On or about May 17, 2013, and at times prior thereto, employees were potenially exposed to the improperly guarded DeWalt DW758 8" bench grinder.  The work rest guard was not adjusted within one-eighth inch from the wheel.

Tool Room - On or about May 17, 2013, and at times prior thereto, employees were exposed to the improperly guarded DeWalt DW758 8 inch bench grinder.  The tongue guard was not adjusted to one-quarter inch from the wheel.

Tool Room - On or about May 17, 2013, and at times prior thereto, the side of the abrasive wheel on the DeWalt brand DW758 8 inch bench grinder was not guarded.  Employees were potentially exposed to the unguarded, rotating spindle end and nut projection during use of the grinder.
