29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about 5/21/13 at residential roofing construction site, 8981 Michael Douglas Dr., Clarence Center, NY: Employees engaged in roofing work were not trained to recognize potential hazards on this type of jobsite including but not limited to falls from roofs, ladders, and scaffolds, electrical hazards, and the need for personal protective equipment. They also were not trained in the applicable OSHA regulations to control or eliminate these hazards. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. a) On or about 5/21/13 at residential roofing construction site 8981 Michael Douglas Dr., Clarence Center, NY: Four employees engaged in roofing work were exposed to a potential falls of approx. 10 to 17 ft. from the "rake edge" and front edge of the garage roof, as well as other areas of the roof. The pitch of the roof was approx. 5 in 12. Slide guards were installed partially along the roof edges in various places, however no guardrail system, safety net system, or personal fall arrest system was being utilized. NO ABATEMENT CERTIFICATION REQUIRED
