29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) On or about 7/27/17 at a roof replacement project located at 9668 Rosecroft Drive, Clarence, New York; employees working on the ground below active roof operations were not wearing hard hats.    NO ABATEMENT CERTIFICATION REQUIRED    Switala's Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 984518, citation number 1, item number 1 and was affirmed as a final order on 9/15/14, with respect to a workplace located at 210 W Main Street, Springville, New York.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) On or about 7/27/17 at a roof replacement project located at 9668 Rosecroft Drive, Clarence, New York; employees performing roofing activities from a ladder jack scaffold approximately 18 feet high were not protected from falling by a personal fall arrest system.    NO ABATEMENT CERTIFICATION REQUIRED    Switala's Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 984518, citation number 2, item number 1a and was affirmed as a final order on 9/15/14, with respect to a workplace located at 210 W Main Street, Springville, New York.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 7/27/17 at a roof replacement project located at 9668 Rosecroft Drive, Clarence, New York; employees working on the roof were replacing roof shingles of a two story house. The roof had a 7 in 12 pitch and eave heights of approximately 20 feet. There was no fall protection in use.    NO ABATEMENT CERTIFICATION REQUIRED    Switala's Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 984518, citation number 2, item number 1b and was affirmed as a final order on 9/15/14, with respect to a workplace located at 210 W Main Street, Springville, New York.
