29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. (a) Arnold Construction, 2195 Stony Hill Rd., Boulder, CO 80305; On and before May 22, 2013, the exposing employer failed to provide adequate fall protective measures to his employees as they worked on a roof which measured approximately 13 foot at the edge, with a 4:12 pitch. Condition exposed employees to fall hazards of 13 feet.

29 CFR 1926.1053(b)(1): When portable ladders where used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; (a) Arnold Construction, 2195 Stony Hill Rd., Boulder, CO 80305; On and before May 22, 2013, the exposing employer failed to ensure that ladder side rails extended at least 3 feet above the upper landing surface to which the ladder was being used to gain access. Condition exposed employees to fall hazards of approximately 13 feet as they used a metal protable extension ladder to gain acccess to the roof on which they were working.
