29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers: (a)Arnold Construction at 306 West Maple Street, Superior, CO 80027: On and before June 8th, 2015, the employer allowed employees to use a blue extension cord connected to an outlet box that was missing its cover. The condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a)Arnold Construction at 306 West Maple Street, Superior, CO 80027: On and before June 8th, 2015, the employer did not ensure that employees were using fall protection when framing a two-story single family home in the following instances: 1)Employees were setting trusses by walking and working on the exterior top plate; 2)The second floor deck on the north side was not guarded; 3)The wall openings, including windows and doorways, were not guarded with conforming guardrails set at 42" for the top rail and 21" for the mid-rail, +/- 3 inches; and 4)The stairway hole on the second floor included framing with studs set 24 inches on center, and was not guarded with conforming guardrails as stated above. The condition exposed employees to fall hazards of up to 20 feet. ARNOLD CONSTRUCTION WAS PREVIOUSLY CITED FOR A REPEAT VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 954558, CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON JANUARY 24, 2014, WHICH BECAME A FINAL ORDER ON FEBRUARY 19, 2014, WITH RESPECT TO A WORKPLACE LOCATED AT 3385 HEIDELBERG DRIVE, BOULDER, CO 80305
