29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) Construction site, County Line Road - On or about May 21, 2013 an employee working in a trench over 14 feet in depth and approximately 40 feet long, did not have a ladder or other safe means of egress from the trench.

29 CFR 1926.652(e)(2)(i): Excavation of material to a level greater than 2 feet (.61m) below the bottom of the members of the support system was allowed:  a) Construction site, County Line Road - On or about May 21, 2013 an employee worked in a trench over 14 feet in depth and approximately 40 feet long installing 30 inch Ductile iron pipe, with the bottom of the trench shield set more than 2 feet above the bottom of the trench.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a) Construction site, County Line Road - On or about May 21, 2013, One fiberglass and two aluminum extension ladders extension ladders with defects and damage including split/splintered side rails, bent/kinked side rails, and broken feet, were not marked or tagged as defective, and had not been withdrawn from service.  Anrich, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.1053(b)(16), which was contained in OSHA inspection number 312295413, citation  number 01, item number 03, and was affirmed as a final order on 06/08/2010, with respect to a workplace located at Clay and Lombard Streets, Shamokin, PA.

29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition):  a) Construction site, County Line Rd. - On or about May 21, 2013, A flagger controlling traffic was provided with a flag that did not meet the requirement of the Manual on Uniform Traffic Control Devices, Millenium edition, Section 6E.03, requiring flags to be at least 24 inches square and made of a good grade of red material.
