29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Laminator #2, the adjustable guard nip point for the embosser rolls had a 1-1/4" gap between the roll and the guard. b) Laminator #2, the nip roller ingoing nip point was not guarded. c) Laminator #1, the adjustable guard for the roller nip point was approximately 3" from the ingoing nip point. d) Laminator #1, the adjustable nip point guard for the emboss rolls was approximately 2" from the ingoing nip point. e) Laminator #1, the front of the wind area was not guarded. there was a distance of approximately 8" from the tray to the ingoing nip point at the roll being formed. f) Cameron winder did not have the roll winding area inrgoing nip point guarded to prevent employee contact. g) Tissue wheels had guarding approximately 63" high that permitted access to the turning wheels while in operation and gates were not interlocked to prevent entry during rotation. h) On Seybold cutter Serial No. 16907, the flywheel and brake assembly were not guarded to prevent access during operation. i) On both conveyors to the Seybold cutters, the inrunning conveyor belt nip points were not guarded on both sides of the conveyors. j) On Baler BS-1, the baler micro switch was not operating correctly during inspection and allowed the baler to operate with the guard gate open.
