29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  a)  On or about January 9, 2019 at 110 Washington Street in Winneconne, employer did not provide written procedure for the Laminator #1 Control of Hazardous Energy; employees were not using lockout and/or tag-out when cleaning machine roller from inside the machine.  b)  On or about January 9, 2019 at 110 Washington Street in Winneconne, employer did not provide written procedure for the Laminator #2 Control of Hazardous Energy; employees were not using lockout and/or tag-out when cleaning machine roller from inside the machine.  c)  On or about January 9, 2019 at 110 Washington Street in Winneconne, employer did not provide written procedure for the Bev Nap machine Control of Hazardous Energy; employees were not using lockout and/or tag-out when removing guards and reaching into machine to clear jams.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.   On or about January 9, 2019 at 110 Washington Street in Winneconne, employer did not provide training in energy control requirements for employees who did not associate laceration and crushing hazards with reaching into machines that did not have power sources isolated from the machine and locked out.  Laminator #1 and #2 machine operators placing body into machine during roller cleaning with machine running and Bev Nap machine operators remove guard and place hands into machine to clear jams with E-Stop used to shut off machine.  The training shall include the following: 1910.147(c)(7)(i)(A)  Recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. 1910.147(c)(7)(i)(B)  Each affected employee shall be instructed in the purpose and use of energy control procedure. 1910.147(c)(7)(i)(C)  All other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which at locked out or tagged out.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a)  On or about January 9, 2019 at 110 Washington Street in Winneconne, employer did not ensure the nip points at the vertical rollers threading area on the Bev Nap machine were guarded; closed double door guard, center gap between edges of doors approximately one inch at top and three and one-half inches at bottom   b)  On or about January 9, 2019 at 110 Washington Street in Winneconne, employer did not ensure rotating part was guarded on the side of the Bev Nap machine near the control area; side opening approximately nine inches W by six inches H with rotating part ten and one-half inches from edge of opening.  c)  On or about January 9, 2019 at 110 Washington Street in Winneconne, employer did not ensure rotating part was guarded on the side of the Bev Nap machine between the control area and edge of installed guard; gap between control box and edge of guard approximately three inches W.

Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.  On or about January 9, 2019 at 110 Washington Street in Winneconne, the employer did not ensure the guards on the Bev Nap Machine for the blade and vertical rollers were secure during operation; guards hung on screw heads at point of operation and on side of machine at control area.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. Required workplace labeling:    1910.1200(f)(6)(i)  The information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or,     1910.1200(f)(6)(i)  Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.
