Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace, including but not limited to, surface conditions where the vehicle will be operated; On or about September 2, 2025, at a workplace located at 110 Washington Street, Winneconne, Wisconsin; the employer did not ensure employees were trained in the specific procedures for securing trailers while unloading enclosure trailers and placement of wheel chokes to prevent trailer movement prior to driving a forklift into a trailer, exposing employees to crushing hazards.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about September 10, 2025, at a workplace located at 110 Washington Street, Winneconne, Wisconsin in the Laminator Department; employees were exposed to in-running nip point on the Morrison Laminator 2, Serial No. 79 101, operator side, created by the unguarded rotating heat roller, chill rollers and the nip roller. b) On or about September 10, 2025, at a workplace located at 110 Washington Street, Winneconne, Wisconsin in the Laminator Department; employees were exposed to thermal hazards on the Morrison Laminator 2, Serial No. 79 101, operator side, created by the unguarded rotating heat roller.
