29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity: Shop: The ground and hot wires of the attachment plug of the 240 volt Hytrol Conveyor Company, Model TR, Serial No. 371380 portable conveyor ( Conveyor) used for the BDS Line #17 were reversed. On or about May 17, 2013, an employee sustained serious bodily injury following a severe electrical shock when made contacts with the Conveyor and the "T" handle of the diverter of the BDS Line #17.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Shop: The noncurrent-carrying metal parts of the 240 volt Hytrol Conveyor Company, Model TR, Serial No 371380, portable conveyor (Conveyor) used for the BDS line #17 were not permanently, continuously and effectively grounded due to reversed designated polarities within the attachment plug of the Conveyor.
