The water pump had been removed from the excavation for the elevator and water had accumulated in the pit.  Adequate precautions were not taken to protect employees from a cave-in hazard due to water accumulation.

The sloping/benching protective system was not constructed in accordance with the 29 CFR 1926.652(b)(1) through 29 CFR 1926.652(b)(4) in that the excavation was not sloped properly for a Type C soil.  In addition, the benches exceeded the height parameters for even a Type B soil allowed by Appendix B of Subpart P of this standard.
