On or about May 24, 2013 - at the above addressed jobsite, an employee was performing material handling operations from a tubular welded frame scaffold, more than twenty five (25) feet above the ground, which had frames that were not joined together vertically by the use of stacking pins, thereby exposing employees to the hazards associated with scaffolding collapse.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about May 24, 2013- at the above addressed jobsite, employees walking and/or working from the roof deck approximately twelve (12) feet above the lower level deck were not protected from falls, thereby exposing the employees to fall hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. a. On or about May 24, 2013 at the above addressed jobsite, an employee performing material handling operations from a tubular welded frame scaffold, more than twenty five (25) feet above the ground was exposed to injuries from falls due to lack of guardrails. THE DREAM WALL, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.451(g)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 190157, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 1, 2012 WITH RESPECT TO A WORKPLACE LOCATED AT 1631-1645 MILWAUKEE AVE., GLENVIEW, IL 60025.
