29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. On or about May 22, 2013, on Rig 42, the stairway landing leading up to the Top Doghouse was not flush with the rig floor, the landing was raised approximately 2-inches above the floor. Employees were exposed to a fall approximately 15-feet from the top of the stairs to the ground.

Employees were exposed to electrical burn and fire hazards.
