29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard.    On or about May 22, 2013, on Rig 42, the stairway landing leading up to the Top Doghouse was not flush with the rig floor, the landing was raised approximately 2-inches above the floor. Employees were exposed to a fall approximately 15-feet from the top of the stairs to the ground.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    On or about May 22, 2013, the 480V electrical panel in engine room was not labeled properly nor marked to indicate the circuits it controlled. Employees were exposed to electrical burn and fire hazards.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:   On or about May 22, 2013, the rig's satellite system and its structure located behind the Toolpusher's trailer was powered with 125-volts but not grounded. Employees were exposed to shock hazards.
