29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons.    On or about May 22, 2013, the Industrial Control Panel on the left side of the Directional Driller Trailer was missing a dead front. Three-600V nominal and three 120 V nominal cables going into the box were exposed. The control box lid was taped opened to the side of the trailer to allow one 600V nominal cable to be spliced into the control box and to not pinch the cord between the box and lid. This condition exposed employees to electrical shock and electrocution hazards.

29 CFR 1910.334(a)(1): Flexible electric cords were fastened with staples or otherwise hung in such a fashion as could damage the outer jacket or insulation:    On or about May 22, 2013, below the Mud Engineers and Directional Driller's Trailer, two 50-foot electrical extension cords were run underneath the trailer floor, the cords were held up with chicken wire. One extension cord showed physical damage made by the chicken wiring cutting into the outer insulation. This condition exposed employees to electrical burn and shock hazards.
