The crew house trailer circuit breaker box had a three prong extension cord spliced into it. This exposed employees to electrical shock hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about February 27, 2014 and at times prior thereto, employees were working on a rig site, HWD Rig #9. The crew house trailer circuit breaker box had a three prong extension cord spliced into it. This same cord had the strain relief damaged. This exposed employees to electrical shock hazards.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables were used where run through doorways, windows, or similar openings: On or about February 27, 2014 and at times prior thereto, employees were working on a rig site, HWD Rig #9. At the water pump located near the driller's trailer, there were six extension cords that were run through the containment cover. Some of the cords exhibited insulation damage. This exposed employees to electrical shock hazards.
