29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: The employer did not establish and implement a written lockout/tagout program for employees exposed to the startup of the screen presses during setup in that on or about March 19, 2013 an employee was carding ink into a screen on Press #4 when the machine was started up and the employee's left arm was pulled back injuring the left elbow, rotator cuff and biceps.
