All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). (a) Employees set up the screen presses, including replacing screens, floodbars and squeegees, without first isolating the electric and pneumatic energy sources. (b) Employees got under the screen presses to adjust the separation distance between the pallet and the screens without first isolating the electric and pneumatic energy sources.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: (a) The employer failed to provide adequate guarding for the screen presses, in that the chain guards were so close to the press, that employees could reach in and over the moving parts and, in some cases the flash units would "shuttle" in and out over the chain guards, thereby exposing employees to being struck by the flash units. (b) Employees operating the screen presses were exposed to rotating machine parts while applying ink to the screens. Atlanta Sportswear, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1060684, citation number 1, item number 1b and was affirmed as a final order on August 14, 2013, with respect to a workplace located at 344 Fox Drive, Piqua, Ohio.
