29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a)  The employer did not provide an eyewash station for employees who added water to batteries for powered industrial vehicles, such as fork lifts and cherry pickers, in the warehouse.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    (a)  The employer did not provide training for employees who were exposed to chemicals, such as but not limited to AlbaChem Spot Lifter II (contains methylene chloride), S.P.I.F. II (eye, skin & respiratory irritant), SAATIChem Remove PW-9 (eye & skin irritant), SAATIchem Remove ER2 (eye & skin irritant) and SAATIChem Remove IR1 (eye & skin irritant).    Atlantis Sportswear, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(1), which was contained in OSHA inspection number 90872,  citation number 2, item  number 4 and was affirmed as a final order on December 23, 2013, with respect to a workplace located at 344 Fox Drive, Piqua, OH 45356.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a)  On or about April 23, 2015, the employer only provided safety glasses for employees to use at the five spray out tables, where signs specifically stated "All employees must wear safety goggles to use the spray out table."
