29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a) In the small machine shop, on or about May 31, 2013 - The rotating blade of an older model Wellsaw Metal Band Saw (MN 1318) was not guarded in that the unused portion of the machine blade was not covered. This condition exposed employees to amputation hazards when performing cut work.     b) In the main shop area, at times prior to May 31, 2013 - Maintenance employees used the Heidenreich & Harbeck Metal Lathe D-35 (MN E30450 SN 80121-6450) to piece work while the machine's chuck was unguarded, exposing employees to caught-in and struck-by hazards.
