29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:        a.  Plating Department -  Workers performing plating and finishing work were required to wear half-face piece type respirators and the employer did not establish and implement a written respiratory protection program in accordance with 29 CFR 1910.134(c)(1) that would describe or include at least the following:    1) Procedures for selecting respirators;  2) Worksite specific procedures;  3) Medical evaluations;  4) Fit testing procedures;  5) Procedures for proper use of respirators in routine and reasonable foreseeable emergency;  6) Procedures and schedules for cleaning, storing, inspecting, repairing and discarding respirators;  7) Employee training regarding respiratory hazards they are exposed to, proper use and limitations of respirators; and  8) Procedures for regularly evaluating the effectiveness of the respirator program.    The respirator used was a 3M half-face elastomeric respirator with 6002 cartridges (TC-84A-4680).  The employer did not ensure that where respirators were required, a written respiratory protection program was established, implemented and maintained.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:         Plating Department - The employer did not provide suitable facilities for immediate emergency flushing of the eyes in the area where injurious corrosive chemicals, such as but not limited to, sulfuric acid and sodium hydroxide, were used by employees.

An employee workplace injury which occurred on 4/24/13 resulted in sutures and 10 days of restricted work activity, and had not been entered on the OSHA 300 log within seven days as required.         b.     An employee workplace injury which occurred on 5/8/13 required two stitches and 12 days of restricted work activity, and had not been entered on the OSHA 300 log within seven days as required.
