Location: paint spray booth  On or about 12/1/15, The Dwyer monometer mounted on the spray booth was inoperative during spray painting operations involving the use of flammable solvents and the filters were clogged with the paint product.

One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    Location: shop area  On or about 12/2/16, the employer did not ensure that employees operating Amada press brakes were protected from hazards created by the point of operation.

Prototek Sheetmetal Fabrication, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.242(b), which was contained in OSHA inspection number 1024668, citation number1, item number 6 and was affirmed as a final order on 04/13/2015 with respect to a workplace located at 244 Burnham Intervale Road, Contoocook, NH 03229.
