29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  (a) on the jobsite - employees were exposed to an 18-ft fall hazard when installing roof decking and fascia without fall protection, observed on or about 5/29/13.   The First Construction Services Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 316306877, citation number 1, item number 1 and was affirmed as a final order on 3/6/2012, with respect to a workplace located at Holmswood Greens, Lithia, FL 33569.     The First Construction Services Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 315484444, citation number 2, item number 1 and was affirmed as a final order on 5/3/12, with respect to a workplace located at 11395 Baymeadows Road E, Jacksonville, FL 32256.     The First Construction Services Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 882403, citation number 2, item number 1 and was affirmed as a final order on 5/31/13, with respect to a workplace located at Building 36 Holly Heath Drive, Tampa, FL 33610.
