Also, the company has been using the attachment without manufacturers prior approval: a) Max General Distribution / 3377 Brookville Street, Hawthorn, PA: On or about May 30, 2013 the Hyster Lift Truck, model S50XM, serial number D187V19006X, did not have an updated data plate when the Cascade (serial #SPR 778849-1 R1) attachment was used. Additionally, the company did not obtain approval from Hyster prior to use of the attachment.
