29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. (a) On or about May 29, 2013 employees were potentially exposed to serious injury from a fall to the lower level of about 20 feet from a hatch opening the was not covered or otherwise guarded when employees were working in the area. (b) On or about May 29, 2013 employees were potentially exposed to serious injury from a fall to a lower level from skylights that were not rated to support human weight and that were not guarded.
