29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee (a) The Mill Casino - On or about April 5, 2019 and at times prior thereto, employees were exposed to falls of 20' while working on the roof of the casino. The Guardian Temporary roof anchors were not installed per manufacture's requirements.

(a) The Mill Casino - On or about April 2, 2019 and at times prior thereto employees were exposed to a fall off the casino roof while operating the Verimat welder on the edge without guardrails or a personal fall arrest system.
