29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: Roof of Mill Casino: The employer at the time of the inspection was not using ground fault circuit interrupters or had not meet the requirements of an assured grounding conductor program. This exposed employees to an electrical shock hazard from not having protection from one of these methods.

29 CFR 1926.502(d)(2) personal fall arrest systems connectors did not have a corrosion finish: Roof of Mill Casino: Fall protection that was on-site and available for employee use had corrosion on the buckles and connectors. This exposed employees to potential falls from not knowing if the corrosion weakened the connectors below the manufactures limits.
