29 CFR 1926.404(b)(1)(ii): Receptacles on a three-wire, dual voltage 120v/240v portable or vehicle-mounted generator rated at 5kW or more, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, were not be protected with ground-fault circuit interrupters:    On or about 6/3/13    Location: 193 Old Tarrytown Road, White Plains, NY 10603    a) Outlets on a Honda 6,500 watt generator used to power an air compressor were not protected by a ground fault circuit interrupter (GFCI).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about 6/3/13    Location: 193 Old Tarrytown Road, White Plains, NY 10603    a) An employee used a 40 foot Werner aluminium extension ladder to access a low sloped roof at a height of approximately 17 feet. The ladder rails extended only 1.5 feet above the landing level.
