On or about June 4, 2013; at the center of the jobsite, the employer allowed employees to operate and work near a mobile crane (Grove-GMK 4090, S/N-4075-9212) without ensuring that the warning labels/decals (swing radius warning/hand signal chart) originally supplied with the equipment by the manufacturer were readable.
