This violation occurred on or about June 6, 2013, in the machine shop area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a Libby turret lathe, Employer ID 203 "Ole Blu"; Hydraulic, Pneumatic, Electrical 480VAC, without written procedures for isolating potentially hazardous energy. b. This violation occurred on or about June 6, 2013, in the machine shop area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a Mazak horizontal manual lathe, Mazak-24 Employer ID 1; Pneumatic, 480VAC Electrical, without written procedures for isolating potentially hazardous energy. c. This violation occurred on or about June 6, 2013, in the machine shop area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a Mazak horizontal manual lathe, Mazak-24 Employer ID 2; Pneumatic, 480VAC Electrical, without written procedures for isolating potentially hazardous energy. d. This violation occurred on or about June 6, 2013, in the machine shop area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a Okuma CNC lathe, Captain L470, S/N 118610; Hydraulic, Electrical 220VAC, without written procedures for isolating potentially hazardous energy. e. This violation occurred on or about June 6, 2013, in the machine shop area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a Mori-Seki CNC lathe, Model# TL-40Y, S/N 36; Hydraulic, Electrical 220VAC, without written procedures for isolating potentially hazardous energy. This violation occurred on or about June 6, 2013 and times prior thereto, where employees were exposed to a struck-by hazard when operating a Libby turret lathe I/D #203 without a guard over the rotating chuck. b. This violation occurred on or about June 6, 2013 and times prior thereto, where employees were exposed to a struck-by hazard when operating an Okuma CNC lathe, Model # Captain L-470, S/N 118610 without the safety door interlock switch in the "on" position. c. This violation occurred on or about June 6, 2013 and times prior thereto, where employees were exposed to a struck-by hazard when operating a Mori Seki CNC lathe, Model # TL-40Y, S/N 36 without the safety door interlock switch in the "normal" position. This violation was observed on or about June 6, 2013, in the West side of the shop where employees were exposed to a struck-by hazard while operating a Dayton Bench grinder with the work rest opening adjusted to approximately 1/2 inch. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that work rests on grinding machinery are adjusted closely to the wheel with a maximum opening of 1/8 inch.

Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the distance between the abrasive wheel periphery and the adjustable tongue or end of the safety guard does not exceed 1/4 inch.
