29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: On or about 6/6/13 Location: 25 South Stone Ave. Elmsford, NY 10523 a) Employees used a Werner aluminium extension ladders that did not extend 3 feet above the landing level of the roof being accessed and used three Werner extension ladders that did not have the rope laynard required to be able to safely raise and lower the fly section. The emplyees were not trained by the employer in the hazards related to ladder use and the procedures to minimize such hazards.
