Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a-#514 Warren Ave, Ho Ho Kus NJ 0723: An employee engaged in gutter installation activities on a 6 on 12 pitched roof was exposed to fall hazards of approximately 18ft to the asphalt below in which a means of fall protection had not been utilized. Condition observed on or about 08/05/2022 A & J Reliable Inc was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(13): which was contained in OSHA Inspection Number 1497548, Citation # 1, Item # 1, issued on 12/22/2020 with respect to a workplace located at 73 Somerset Drive, Suffern NY 10901. This citation became a final order on 1/08/2021.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. a-#514 Warren Ave, Ho Ho Kus NJ 0723: An employee engaged in gutter installation activities using a portable extension ladder was exposed to fall hazards of approximately 18ft to the asphalt below in which the ladder side rails did not extend 3ft above the upper landing surface. Condition observed on or about 08/05/2022 A & J Reliable Inc was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.1053(b)(1): which was contained in OSHA Inspection Number 1497548, Citation # 1, Item # 2, issued on 12/22/2020 with respect to a workplace located at 73 Somerset Drive, Suffern NY 10901.
