29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a monitoring system: a) Universal Enterprises Inc. at 1784 Steel Street, Louisville, CO 80027: On or about June 8, 2013, the employer did not ensure that three employees were protected from falling when laying TPO on six decks that were each 3 feet wide and 11 feet long. Condition exposed employees to fall hazards of 10 feet 8 inches.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Universal Enterprises Inc. at 1784 Steel Street, Louisville, CO 80027: On or about June 8, 2013, the employer did not ensure that three employees were adequately trained on the chemical hazards of the TPO-C bonding glue. This Mule Hide Products # 308962 bonding glue contained Toluene, Heptane, Acetone, and Xylene. Condition exposed employees to chemical hazards.
