29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  On or about June 10, 2013 and times prior thereto, employees of Lawson Contracting, LLC performed decking operations on a 9:12 pitch steep sloped roof during the construction of the First Security Bank located at or near 10 Bass Pro Lane in Little Rock, Arkansas.  An employee was observed cutting plywood for decking operations while standing on plywood material stacked on raised forks of a GEHL RS8-42 telescopic material handler.  The working platform was not approved by the manufacturer for use, exposing the employee to falling hazards of approximately 14 feet to the ground level below.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee.  On or about June 10, 2013 and times prior thereto, employees of Lawson Contracting, LLC performed decking operations on a 9:12 pitch steep-sloped roof during the construction of the First Security Bank located at or near 10 Bass Pro Lane in Little Rock, Arkansas.  An employee was observed cutting plywood for decking operations while standing on plywood stacked material on raised forks of a GEHL RS8-42 telescopic material handler.  The employee wore a personal fall arrest system and was tied-off using a rope lifeline.  The anchor point for attachment of the rope lifeline was on the carriage of the lift of the telescopic material handler used to support the work platform, exposing the employee to fall hazards in the event the GEHL RS8-42 telescopic material handler used had tipped-over.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about June 10, 2013 and times prior thereto, employees of Lawson Contracting, LLC performed decking operations on a 9:12 pitch steep sloped roof during the construction of the First Security Bank located at or near 10 Bass Pro Lane in Little Rock, Arkansas.  The lifelines of the personal fall arrest systems used were rigged such that the employees would fall greater than 6 feet to the ground level below.
