29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: The employer does not ensure all scaffold platforms are at least 18 inches wide. This violation occurred on or about September 16, 2014, on the south facing side of a commercial building being constructed, when an employee was exposed to a fall hazard while working on a scaffold platform that was not 18 inches wide. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffolds are constructed an maintained in accordance with 29 CFR 1926.451 or 29 CFR 1926.452.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: The employer does not ensure employees are protected from falls while working on scaffolds. This violation occurred on or about September 16, 2014, on the south facing side of a commercial building being constructed, when an employee was exposed to a fall hazard while working on carpenters' bracket scaffold without a guardrail system or personal fall arrest system in place. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffolds used by employees have a guardrail or personal fall arrest system installed that protects employees from falling from all open sides and ends of the platform.
