29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means:    a) On or about 6/8/13 at the Greenpac Construction Project located at 4400 Royal Avenue, Niagara Falls, New York; employees performing site preparation for a concrete driveway were excavating to the identified depth for the apron of the driveway. A service tap on an underground natural gas line was struck and knocked off by the CAT315C Excavator while digging to a depth of 8 inches for the apron at the intersection of the street.    NO ABATEMENT CERTIFICATION REQUIRED
