29 CFR 1926.350(a)(9): Compressed gas cylinders were not secured in an upright position at all times except for short periods of time while cylinders are actually being hoisted or carried.  a) On or about 10/06/15, on Bridge over Railroad Tracks construction site at Ridge Rd., West Of Rosary Ave., Lackawanna, NY: A full oxygen cylinder and a full acetylene cylinder were laying horizontally on the grass right next to each other.   NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On or about 10/06/15, on Bridge over Railroad Tracks construction site at Ridge Rd., West Of Rosary Ave.,Lackawanna, NY: A ladder jack scaffold was erected at the block wall, on the west side of the railroad tracks, at the south side of the bridge. An employee was on the aluminum pik of the scaffold, using a Stihl Cutting Saw to cut into the concrete wall and was exposed to a fall of approx. 16 ft. to the ground from the north and south ends of the scaffold platform. The east side of the scaffold had a proper guardrail consisting of a top rail and a mid rail, but the north and south ends had only a top rail. No personal fall arrest system was utilized.   NO ABATEMENT CERTIFICATION REQUIRED
