The extension cord the employees handle, is damaged to the outer insulation was not properly repaired. The damaged electrical cord is being used in adverse conditions, on or about 06/17/13.

29 CFR 1910.305(a)(2)(x): Flexible cords were not protected from accidental damage, as might be caused for example, by sharp corners, projections, and doorways or other pinch points: a. 823 Winer Industrial Way, Lawrenceville, GA (Outside) - Employees were exposed to an electrical hazard. An extension cord was not protected from accidental damage due to a pinch point area. The employees handle the flexible cord, which is used as an extension cord, to provide power to the sump pump. The cord was run underneath the closed garage door and was not protected from damage while pinched under the door, on or about 06/17/13.
