29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): a. On or about November 22, 2024, the employer failed to provide effective guarding to protect employees working at or near the #2 Smart Products 12" blade, chop saw, from a rotating belt and pulley while cutting pallet deck boards. A saw operator standing in front of the saw to make cuts was exposed to caught in hazards from rotating parts during normal production operations when their hands were approximately 18" away from and unguarded belt and pulley. b. On or about November 22, 2024, the employer failed to provide effective guarding to protect employees working at or near the #3 Smart Products 16" blade saw a rotating belt and pulley while cutting stringer boards. A saw operator standing in front of the saw to make cuts was exposed to caught in hazards from rotating parts during normal production operations when their hands were approximately 18" away from an unguarded belt and pulley.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section. a. On or about November 22, 2024, the employer failed to provide effective guarding to protect employees working at or near the #2 Smart Products 12" blade chop saw from a rotating belt and pulley while cutting wooden pallet deck boards. A saw operator standing in front of the saw to make cuts was exposed to rotating parts during normal production operations when their hands are approximately 18" away from and unguarded belt.

On or about November 22, 2024, at the #2 Saw Products chop saw, the employer failed to ensure the flexible cord attached to the flexible cord attached to the 220-volt Marathon Electric motor, Model - FVJ 184TTPR4001BCL, was provided with strain relief that would prevent pull from being transmitted directly to electrical joints or terminal screws, therefore exposing a saw operator to electric shock hazards.
