29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.     On or about June 19, 2013, at the above addressed jobsite, employees were performing roofing operations from a ladder jack scaffold with the platform over 20 feet above the ground, the platform extended more than 18 inches past the support.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.     On or about June 19, 2013, at the above addressed jobsite, an employee conducting roofing operations from a ladder jack scaffold, 20 feet above the ground, was not provided with an access ladder, thereby exposing the employee to falls.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about June 19, 2013, at the above addressed jobsite, an employee was performing roofing work from a platform that was approximately 12.2 feet above  ground. The employee was exposed to injuries from falls due to the lack of guardrails.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
