29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Jobsite: The employees, who were working from the wooden platform that was placed on top of the guardrail of a scissor lift platform at a height of approximately 16 feet 5 inches above the ground were not protected from fall hazards by the use of guardrails or personal fall arrest systems.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees:  Jobsite:  The employees, engaged in cutting and laying out one-half (½) inch roofing sheathing placed sheets of wood on top of the guardrail of a scissor's lift platform to increase the height of working level.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      Jobsite:  The employees who were working on the 8 pitch roof at a height of 12 fft 11 inches from the eave to the ground were not protected from fall hazards by the use guardrails, safety nets or personal fall arrest systems.      J. J Reira Construction, LLC was previously cited for a violation of an equivalent occupational safety and health standard  29 CFR 1926.503(b)(13), which was contained in OSHA inspection number 313626004, citation number 1, item number 1a and was affirmed as a final order on 06 April 2010, with respect to a workplace located at 477 Main Street Monroe CT 06468.
