29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     a) Employees walking on top plate of the North and South wall frame setting joist without fall protection exposed to a fall of approximately 14 feet, on or about 06/25/13.     b) Employees working around window openings with a height to the sill of 29 inches on the East, West and South walls without fall protection exposed to a fall of approximately 12 feet, on or about 06/25/13.     c)  Employees working around the opening to the stairs, on the North side, were exposed to an approximately 8 foot fall without fall protection, on or about 06/25/13.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1):  The side rails of portable ladders used for access to upper landing surfaces, did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder.     a) Employees were using a Werner 6 foot step ladder on the west wall which did not extend over the ledge to access the top plate of the wall frame exposing them to a fall of approximately 14 feet to the exterior ground and a 9 foot fall to the interior floor, on or about 06/25/13.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. 29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, was not immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and was not withdrawn from service until repaired.     a) Employee was using on the east side of the house a Werner 8 foot step ladder with a support brace broken off, to reach the joist exposing him to a fall should the ladder become unstable, on or about 06/25/13.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
