29 CFR 1926.851(b): All stairs, passageways, ladders and incidental equipment thereto, which are covered by this section, were not periodically inspected and maintained in a clean and safe condition. a) Stairway Between Second and Third Floors - An employee was using a stairway that was 40 inches wide, with 12 risers without a handrail or stair rail on the unprotected side and employees exposed to a 9 feet, 7 inch fall hazard, on or about 08/09/2017. For the purpose of this inspection, this employer was determined to be the controlling and correcting employer on the job site. The employer is being cited under OSHAs multi-employer workplace policy. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Third Floor - An employee was observed removing a brick wall where the top of the brick wall was 18 inches above the floor and exposed to a 37 foot fall hazard, on or about 08/09/2017. b) First Floor - Employees were standing near a bank of window openings that were 30 inches wide, 6 feet in height,14 inches from the sill to floor and exposed to a fall of 12 feet, 4 inches, on or about 08/09/2017. Quality Carpentry Group Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 935203, citation number 1, item number 1, and was affirmed as a final order on 04/07/2014, with respect to a workplace located at 1335 Maryland Avenue, NE Washington DC 20002. For the purpose of this inspection, this employer was determined to be the controlling and correcting employer on the job site. The employer is being cited under OSHAs multi-employer workplace policy. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
