29 CFR 1910.146(d)(6): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide at least one attendant outside the permit space into which entry was authorized for the duration of entry operations:    (a)  On or about June 24, 2013 and at times prior, for employees exposed to potentially hazardous atmosphere inside of a 30,000 gallon tank while performing lay up belly banding operations using a styrene based resin at Design Tanks LLC at 612 West Blackhawk Street in Sioux Falls, South Dakota.

29 CFR 1910.146(d)(5)(i): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not evaluate permit space conditions when entry operations were conducted by testing conditions in the permit space to determine if conditions were acceptable:    (a)  On or about June 25, 2013 and at times prior for employees exposed to a potentially hazardous atmosphere inside of a confined space while chopping fiberglass and styrene based resin inside forms approximately five feet high by five feet wide by eight to ten feet long at Design Tanks LLC at 612 West Blackhawk Street in Sioux Falls, South Dakota.    Note:  This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 524879, Citation 1, Item 1A, and was affirmed as a final order on January 25, 2013 , with respect to a workplace located at 612 West Blackhawk Street in Sioux Falls, South Dakota.
