The pump pits (Building 340) located near Building 8341 was not labeled with danger signs or any other equally effective means indicating the existence, location, and danger posed by these permit spaces. c. The holding tank near Building 35 was not labeled with danger signs or any other equally effective means indicating the existence, location, and danger posed by these permit spaces.

29 CFR 1904.29(b)(3) as required by 1960.8(b): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301 or equivalent) within seven (7) days of receiving information that a recordable injury or illness has occurred: At the Selfridge Air National Guard Base, the employer failed to ensure that an employee's work related standard threshold shift was recorded on the OSHA 300 log within seven (7) days as required by 1904.10(b)(4). The retest was conducted on April 12, 2013 and not recorded until May 2, 2013.
