29 CFR 1910.147(c)(6)(i) as required by 1960.8(b):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   (a) Civil Engineering, Utilities,  the employer did not conduct an annual or more frequent inspection of the energy control procedure by an authorized employee to ensure that they were properly implemented during repair and service work on equipment.  (b) 191 MXS/MXMFS, Sheet Metal Shop,  the employer did not conduct an annual or more frequent inspection of the energy control procedure by an authorized employee to ensure that they were properly implemented during repair and service work on equipment. 29 CFR 1910.147(c)(7)(i) as required by 1960.8(B): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:   Civil Engineering, Vehicle Maintenance, the employer did not provide training to "authorized" and/or "affected" employees on applicable hazardous energy sources, types and magnitude of energy, and the means and methods of isolating and/or controlling energy, and the means of verification of effective energy control, and the purpose of the procedure to be used. "Other" employees who worked in the area where the energy control procedures were utilized, were not trained on the lockout / tag out procedures and the prohibition against attempting to restart or reenergize equipment that was locked or tagged out. Employees were exposed to mechanical, electrical and hydraulic energy hazards while performing maintenance, servicing and repair work on machines or equipment such as, but not limited to, vehicle lifts.

(b)  Hangar 36, Metal Technologies Shop, Clausing 600 Metal Lathe, S/N VM0806, point of operation was not equipped with a guard during operation, exposing employees to struck by hazards from flying objects.  (c)  Building 139, Vehicle Maintenance Shop, Leblond Regal Lathe, S/N B5499, point of operation was not equipped with a guard during operation, exposing employees to caught in and/or struck by hazards.

29 CFR 1910.212(a)(1) as required by 1960.8(b):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a)  Hangar 36, Sheet Metal Shop,  Band Saw, Model # 3618TS, S/N 3868-95, The non-working portion of the band saw blade was not guarded or enclosed exposing employees to an amputation hazard.    (b)  Building 139, Vehicle Maintenance Shop, Cutoff Band Saw, Model # V4014, S/N A1967V8, The non-working portion of the band saw blade was not guarded or enclosed exposing employees to an amputation hazard.   The 168th Air Refueling Wing, Alaska Air National Guard was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 823981, citation number 1, item number 1 and was affirmed as a final order on February 26, 2013, with respect to a workplace located at Eielson Air Force Base, Alaska.
