29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:    (a)  Employees operating the Monarch Vertical Machine Centers, the Haas Vertical Machine Center, and the Hardinge Turning Centers, were not provided with adequate training on the energy control program, in that they performed setups and cleaned the inside of the machines without first isolating and locking out the disconnects.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    (a)  Employees set up and cleaned the Monarch Vertical Machine Centers, the Haas Vertical Machine Center, and the Hardinge Turning Centers, without first isolating the energy sources. 29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc:    (a)  The point of operation was not guarded on the Monarch Vertical Machine Center (S/N 83311), Monarch Vertical Machine Center (S/N 85347), and the Monarch Vertical Machine Center (S/N 86346).    (b)  The point of operation guards on the Haas Vertical Machine Center (S/N 14640), the Hardinge Turning Center (S/N SB-379), the Hardinge Turning Center (S/N CN-2860-S), the Hardinge Turning Center (CN-2074-A2-16), and the Hardinge Turning Center (CN-2087-A2-16) were not interlocked.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws:    (a)  The outer insullation was pulled away, exposing the inner insulated wires where they entered the plug, from a drop down cord that was connected to a floor fan, thereby exposing employees to 110-VAC.
