29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system: a) Utility Work Site: On or about July 1, 2013 and at times prior, as the controlling employer Martel Construction, Inc., did not ensure the employees of its subcontractor, Huppert Construction Company, Inc. were protected from cave in hazards while working in an unprotected trench approximately 6 feet deep installing storm drain pipe.
